Nova Scotia African Nova Scotian Health Equity Readiness Assessment

Nova Scotia requires organizations to demonstrate operational readiness before funds address health disparities affecting the 22,000 African Nova Scotian residents concentrated in Preston, North Preston, East Preston, and Cherry Brook. Historical underinvestment has produced life-expectancy gaps of 5.2 years compared with provincial averages, driven by documented differences in hypertension and diabetes management.

Readiness documentation includes current partnerships with the African Nova Scotian Affairs office and proof of community-governed advisory boards that have met at least quarterly for the preceding 18 months. Infrastructure constraints include limited clinic hours in rural HRM districts and absence of electronic health record templates incorporating culturally specific dietary and family-history fields.

Applicants must submit facility accessibility audits confirming physical space for group sessions and transportation coordination with existing community vans. Workforce plans require at least 40 percent of proposed staff to hold existing ties to African Nova Scotian communities or equivalent lived-experience credentials.

Funding release is contingent on letters of support from the Nova Scotia Health Authority's African Canadian Health Equity team and alignment with the province's 2022 Dismantling Racism and Hate Act reporting requirements. This structure differs from other Maritime submissions by mandating pre-existing governance structures rather than allowing formation during the grant period.
